TicketContent custom fields add field in tree view

Hello,

I created a Ticket custom field with a combobox, but I am absolutely unable to add it under my tree view.
It seems that I need first to add a label? But how can I do that?

In the following picture, the arrow is where I want to place my custom field:

To piggy back off of this topic, I want to put a custom field in a combobox list with multiple options but I can only put a custom field in once. How do I overcome this?

For example:

Wrapper container (Combobox)
* Combobox item 1
** custom field box
* Combobox item 2
** custom field box
