When I right-click in the All Reports section to add a new report, nothing happens. Also, under All Reports, how can I force these items to scan again? For instance, the item for Not Latest Service Pack XP has outdated info.

What is the value for dbo.TblOperatingSystem.Version for Windows XP? The report I have actually has it blank.

Lower the waittime for "OS" to 0.

first check if the report is correct (servicepack=3)

OK that worked - do you know how I can fix the data for the Not Latest Service Pack XP?

You are trying to create a report with the configuration utility.
You need a seperate tool : the report builder.

This comes in the premium pack as lsbuilder.exe

I know how to create a report, at least based off the documentation, but when I right-click, there is no context menu to "Add New" For instance, in the attached "Capture1.JPG", I try to right-click on All member servers, or on the All available reports tab, and I don't get that option.

The option I'm looking for is what is in Capture2, as I don't have that screen when I right-click

For the scan options, as you can see from a partial screen shot for (capture3.jpg), all items are set to 1 day

What I'm trying to update, is in capture4, the value for the Not Latest Service Pack for Windows XP

I'm not sure what you mean by this, could you attach a screenshot please.

Check the documentation for page "Configuring scanning options"
You probably need to lower the waittime.
