Cleanup or modification of existing asset records with same IP, same MAC does not occur; instead, LS creates a 2nd asset with the new information. Does the "Detect when a computer has been renamed" setting have to be consistent across all servers?

Computer renaming detection needs to be enabled on all scanning servers if you have multiple ones. If it is enabled, the MAC address, model and serial number will be used to compare newly scanned Windows computers with existing database entries. If one of these does not match, Lansweeper generates a new asset page. Possibly this explains your issue. If computer renaming detection is not enabled, Lansweeper creates a new asset page for every Windows computer which has a different domain/hostname combination.
For non-Windows assets, Lansweeper will update existing assets if the same MAC address was scanned on another IP.
Details on how Lansweeper uniquely identifies assets can be found in this KB article.
